CHECKLIST OF REQUIREMENTS FOR ALL BANKRUPTCY CLIENTS

Completed Bankruptcy Questionnaire (entirely)
Certification from an approved bankruptcy counseling prior to filing of bankruptcy
Pay stubs or other proof of income for the past 6 months
Bank statements for the past 6 months
List all personal property and provide information on spouse on pages 1 and 12-26
Ch. 7 debtors filing individually must provide information on spouse on pages 12-15
In any joint case, complete information on both husband and wife
The account number, address, and amount owed must be provided for each creditor
